This website is published by Life Medical, a medical practice in New Jersey, to explain the Care Navigator Program and Principal Illness Navigation. By using the site you agree to these terms.

Not medical advice

The information here is general and educational. It is not medical advice and does not create a patient–practitioner relationship. Always speak with your practitioner about your own care. In an emergency, call 911.

Eligibility and coverage

Principal Illness Navigation is a covered Medicare service. Your practitioner determines whether you qualify. Part B cost sharing may apply and is waived with full Medicaid or QMB. Nothing on this site is a guarantee of coverage, payment, or a clinical outcome.

Staff portals

The employee portal and the mobile time log are for authorized Life Medical staff only. Access is logged. Unauthorized use is prohibited.
