HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed, and how you can get access to this information. Please review it carefully. Last updated August 2026.

Our commitment

Life Medical is required by law to protect the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

Treatment. Your care navigator, practitioner, and care team use your information to coordinate your care, arrange appointments and services, and communicate with the other providers involved in treating you.

Payment. We use your information to bill Medicare and any secondary plan for Principal Illness Navigation and other covered services, including confirming coverage and documenting the time and activities required for a claim.

Health care operations. We use your information for quality review, training, compliance, and running the program.

As required or permitted by law. This includes public health reporting, health oversight, legal process, preventing a serious threat to health or safety, and other uses the law requires.

Uses that require your written authorization

We never sell your health information and never use it for marketing or advertising. Most uses of psychotherapy notes and any other use not described here require your written authorization, which you may revoke at any time in writing.

Your rights

You have the right to see and get a copy of your record, to ask us to correct it, to ask for confidential communication at an alternate address or phone number, to request a restriction on certain uses and disclosures, to get a list of certain disclosures we have made, and to get a paper copy of this notice.

How to exercise a right or file a complaint

Call 973-607-4911 and ask for the Privacy Officer. You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. We will never retaliate against you for filing a complaint.

Changes to this notice

We may change this notice and apply the change to information we already hold. The current version is always posted on this page.
