The people who navigate your care

Principal Illness Navigation only works when a real, named person owns it. At Life Medical every patient in the program is assigned a licensed navigator who works alongside the practitioner who set the diagnosis and the care plan. You keep your own doctors. The navigator handles everything between the visits.

Care Navigator

Mia is a Licensed Practical Nurse and the Lead Navigator for the Life Care Navigator Program. She is the person most patients speak with first, and the one who stays with them month to month.

Practitioners

Every patient in the program has a billing practitioner, a physician or advanced practice clinician, who confirms the qualifying condition, signs the treatment plan and reviews the navigator's work each month. Life Medical also has its own doctors and clinicians who cover gaps in care and are there for anything urgent.

Getting started

How to start

From your first call to your first month, this is the whole process.

  1. You call us

    Tell us what is going on. We check your Medicare coverage, your condition and your visit history while you are on the phone.

  2. Your doctor orders it

    The service starts with a visit with a Life Medical practitioner. If you have not had one this year, we book it, and we can arrange the ride.

  3. You meet your Navigator

    A welcome call in the first week, with your family if you want them there. You go through your medications, your appointments, your worries and what matters most to you.

  4. You get a plan, and a person

    By the end of the first month you have a written care plan and a direct number. You know who to call when you do not know who to call.
