Life Medical is a medical practice in New Jersey. We run the Care Navigator Program, which delivers Principal Illness Navigation, a covered Medicare service. This policy explains how we handle information collected through this website and by phone.

Health information

Health information you share with us as a patient is protected health information under HIPAA. It is held in our clinical systems, used to coordinate your care and to bill Medicare, and shared only with your care team, your practitioner, and those permitted by law. It is never sold and never used for advertising. Our Notice of Privacy Practices governs that information and takes precedence over this policy.

To answer your questions, confirm eligibility, coordinate your care, meet Medicare documentation requirements, and improve this website. We do not sell personal information.

This site uses a simple first-party page counter. It does not build advertising profiles and does not follow you across other websites.

How long we keep it

Medical and billing records are kept for the period required by New Jersey and federal law. Website analytics are kept in aggregate.
